LatAm-FINGERS builds directly upon the foundational insights gleaned from the U.S. Study to Protect Brain Health Through Lifestyle Intervention to Reduce Risk (U.S. POINTER), further validating the efficacy and adaptability of multi-domain lifestyle interventions. These comprehensive programs, typically integrating elements of physical activity, nutritious eating, cognitive stimulation, and social engagement, have now demonstrated their capacity to be successfully implemented and yield positive outcomes across vastly different cultural landscapes, healthcare infrastructures, and community settings.
The global prevalence of dementia is on an alarming upward trajectory, posing an immense public health and socioeconomic burden worldwide. Regions like Latin America, characterized by diverse populations and often varying access to resources, are particularly vulnerable to the projected increase in dementia cases. This escalating crisis has spurred researchers and public health advocates to seek effective, scalable, and affordable preventative strategies. The World-Wide FINGERS network, from which LatAm-FINGERS originates, was established precisely to address this need. Its genesis lies in the pioneering Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability (FINGER) trial, which first demonstrated that a multi-pronged approach targeting several modifiable risk factors simultaneously could preserve cognitive function in at-risk older adults. This revolutionary concept, now globally adopted, posits that since Alzheimer’s disease and other dementias are influenced by a confluence of health and lifestyle factors, a holistic intervention addressing these elements concurrently offers the most promising avenue for prevention.
Dr. Lucia Crivelli, the lead author of the LatAm-FINGERS study and a principal investigator at Fleni, a distinguished neurological institute based in Buenos Aires, Argentina, emphasized the transformative nature of these results. "For the first time within Latin America, our findings definitively illustrate that lifestyle interventions, thoughtfully adapted to local cultures, can be effectively deployed across a spectrum of diverse nations and communities," Dr. Crivelli stated. The LatAm-FINGERS analysis encompassed 1,065 individuals recruited across 12 study sites located in Argentina, Bolivia, Brazil, Chile, Colombia, Costa Rica, Dominican Republic, Ecuador, Mexico, Peru, and Uruguay. Participants were randomly assigned to one of two distinct intervention groups, differentiated primarily by the intensity, structure, and ongoing support provided by the program.
The Systematic Lifestyle Intervention (SLI) group comprised 539 participants. This arm received an intensive, continuous regimen of coaching and comprehensive support. Key components included supervised exercise sessions, personalized nutrition counseling based on the modified MIND diet, engaging computerized cognitive training exercises, vigilant monitoring of cardiovascular risk factors, and participation in 38 structured group meetings. These meetings were specifically designed to foster social connection among participants and cultivate a strong sense of accountability, both crucial for sustained behavioral change.
In contrast, the Flexible Lifestyle Intervention (FLI) group consisted of 526 participants. This arm received periodic health education alongside more general lifestyle recommendations. Over the two-year study period, these participants attended only four group meetings, which broadly covered topics such as diet, physical activity, cognitive and social engagement, and vascular risk management. Crucially, unlike their counterparts in the SLI group, individuals in the FLI group did not benefit from continuous coaching or ongoing supervision, serving as a less intensive control for the more rigorous SLI approach.
After the two-year intervention period, the results clearly demonstrated the superiority of the more structured SLI program in fostering cognitive gains. Participants in the SLI group exhibited a remarkable 55% greater improvement on a composite measure of global cognition when compared to those in the FLI group. Beyond this overarching cognitive benefit, the SLI participants also experienced significantly larger improvements in specific cognitive domains, including memory recall, executive function (encompassing planning, problem-solving, and decision-making), and processing speed, which refers to the rate at which an individual can take in and respond to information.
