A groundbreaking study conducted by researchers at the School of Public Health, LKS Faculty of Medicine, The University of Hong Kong (HKUMed), has unveiled a potentially transformative approach to managing central obesity, demonstrating that a single, vigorous interval walking session per week can deliver significant reductions in body fat and notable improvements in cardiorespiratory fitness. These outcomes were found to be remarkably similar to those achieved by individuals adhering to a more demanding thrice-weekly exercise schedule, a frequency long considered the benchmark in conventional fitness guidance. This research suggests that a concentrated, once-a-week regimen of brisk interval walking could serve as a practical and attainable intervention for adults struggling with excess abdominal fat, particularly those facing significant time constraints that impede their ability to engage in multiple exercise sessions. The comprehensive findings of this investigation have been formally published in the esteemed scientific journal, Nature Communications.
The accumulation of adipose tissue, particularly in the abdominal region, represents a significant public health concern, extending beyond aesthetic considerations to encompass a heightened risk profile for a spectrum of serious health complications. Visceral fat, the type that encircles internal organs, is intrinsically linked to an increased propensity for cardiovascular ailments, the development of metabolic dysfunctions such as type 2 diabetes, and an elevated likelihood of premature mortality. While the indispensable role of physical activity in the holistic management of obesity and its associated comorbidities is well-established, the persistent challenge for a substantial portion of the population, especially those most affected by obesity, lies in sustaining a consistent pattern of regular exercise over the long term. This adherence barrier is often rooted in a confluence of factors, including demanding professional obligations, familial responsibilities, and the sheer lack of available time.
Interval training, characterized by its alternating periods of high-intensity exertion followed by brief intervals of active recovery, has emerged as a highly effective strategy for enhancing fitness and promoting fat loss. Scientific evidence indicates that this method can be more time-efficient in reducing overall body fat and, crucially, visceral fat, when contrasted with sustained moderate-intensity exercise. However, the prevailing exercise guidelines typically advocate for interval training to be distributed across three sessions each week. This frequency, while beneficial, presents a formidable obstacle for many individuals whose daily lives are already replete with commitments, leaving little room for multiple dedicated workout slots.
In light of these adherence challenges, the concept of concentrated exercise patterns, often colloquially termed "weekend warrior" behavior, has garnered increasing attention. This approach involves consolidating the majority of weekly physical activity into one or two days. While prior research has hinted at the potential health benefits of such a strategy, empirical data specifically examining the efficacy of interval training when confined to a single weekly session has been relatively scarce. Professor Parco Siu Ming-fai, a distinguished figure and Head of the Division of Kinesiology at the School of Public Health, HKUMed, recognized the critical need to investigate whether a singular weekly interval training session could indeed rival the positive outcomes of a traditional three-session schedule, thereby offering a more accessible and practical alternative for a broader segment of the population.
To rigorously assess this hypothesis, the HKUMed research team orchestrated a meticulously designed randomized controlled trial, which commenced in September 2021 and concluded in September 2024. The study enrolled 315 adult participants residing in Hong Kong, all of whom were aged 18 years or older, presented with overweight conditions, and exhibited indicators of central obesity. Participants were strategically allocated into one of three distinct groups. The first group was assigned to engage in interval training precisely once per week. The second group was tasked with performing interval training three times per week, aligning with conventional recommendations. The third group served as a control, receiving educational health information instead of structured exercise.
The control group participated in a bi-weekly, 2.5-hour health education seminar over a period of four months. In contrast, both experimental exercise groups were instructed to complete a total of 75 minutes of interval training per week. The key distinction lay in the delivery of this training: one group performed the entire 75 minutes in a single, consolidated session, while the other group divided the total weekly duration across three separate exercise sessions.
To objectively quantify the impact of these differing exercise regimens on body composition, researchers employed dual-energy X-ray absorptiometry (DXA), a highly accurate method for measuring bone density and soft tissue composition. These measurements were taken at three critical junctures: at the outset of the study (baseline), following the 16-week intervention period, and again at the 32-week mark, which represented a four-month post-intervention follow-up phase, allowing for an assessment of sustained effects.
The findings emerging from the 16-week assessment were compelling and provided clear evidence of the efficacy of both interval training schedules. Participants in both the once-weekly and thrice-weekly interval training groups demonstrated statistically significant and comparable improvements across several key health metrics. Specifically, both exercise cohorts experienced a reduction in total body fat mass, a decrease in overall body fat percentage, and a narrowing of waist circumference. Furthermore, compared to the control group, individuals in both interval training groups exhibited a marked enhancement in their cardiorespiratory fitness, a vital indicator of cardiovascular health and exercise capacity.
Professor Siu elaborated on the significance of these results, stating, "While thrice-weekly interval training continues to be a widely endorsed strategy for the therapeutic management of excess adiposity, our findings conclusively demonstrate that concentrating interval training into a single weekly session offers comparable physiological benefits. This presents a highly practical and accessible exercise paradigm." He further emphasized the pervasive issue of time scarcity, noting, "For a considerable number of adults grappling with central obesity, the intricate balancing act between professional duties, academic pursuits, family obligations, and other life commitments renders the prospect of exercising on multiple days each week an insurmountable barrier."
Professor Siu concluded by advocating for a broader perspective in exercise prescription: "Rather than solely relying on recommendations for high-frequency exercise, the viability and effectiveness of once-weekly interval training as a standalone intervention should be seriously considered. This approach offers a tangible solution for individuals who are motivated to improve their health but are constrained by time limitations."
The pivotal research was spearheaded by Professor Parco Siu Ming-fai, who holds the esteemed positions of Professor and Head of the Division of Kinesiology within the School of Public Health at HKUMed. The study’s co-first authors, Dr. Leung Chit-kay and Mr. Joshua Bernal, also affiliated with the same academic institution, played instrumental roles in its execution. The scientific endeavor received crucial financial backing from the General Research Fund of the Research Grants Council, an initiative of the University Grants Committee of Hong Kong, China, as well as the Seed Fund for Basic Research at HKU, underscoring the collaborative and well-supported nature of this significant research.
